What Is the DOT RTD Process? The Return-to-Duty (RTD) process is a federally mandated program that must be followed when a DOT-regulated employee violates drug and/or alcohol regulations. A violation may occur due to a positive test result, a refusal to test, or any other DOT-defined infraction. The RTD process includes: Initial Evaluation by a Substance Abuse Professional (SAP):
The employee must be evaluated by a DOT-qualified SAP who will determine the necessary education or treatment. Completion of SAP Recommendations:
The employee must complete the program (education or treatment) recommended by the SAP. Follow-up Evaluation:
The SAP will re-evaluate the employee to ensure compliance.

Return-to-Duty Test:
A directly observed drug and/or alcohol test must be taken and passed before the employee can return to safety-sensitive duties.

Follow-Up Testing Plan:
The SAP will prescribe unannounced testing (minimum of six tests in the first 12 months), which can last up to 60 months. DOT Drug Testing Requirements DOT drug testing is required under the following circumstances: Pre-employment Random testing Reasonable suspicion Post-accident Return-to-duty Follow-up Substances tested include marijuana (THC), cocaine, opioids, amphetamines, and PCP. All drug tests are conducted using urine samples analyzed by a certified lab, such as RPT Labs, ensuring accuracy and compliance. DOT Alcohol Testing Requirements DOT alcohol testing is conducted using breath or saliva and applies to: Random alcohol tests Reasonable suspicion Post-accident Return-to-duty Follow-up A blood alcohol concentration (BAC) of 0.02 or higher can trigger consequences, with 0.04 or higher considered a failed test. FMCSA Drug & Alcohol Clearinghouse Reporting The FMCSA Clearinghouse is an online database that tracks drug and alcohol program violations for CDL drivers. Employers and Medical Review Officers (MROs) are required to report: Positive drug test results Alcohol test results of 0.04 or higher Test refusals Actual knowledge of violations Completion of RTD steps Employers must conduct pre-employment queries and annual checks on all current CDL drivers through the Clearinghouse.
